Job Title: Advanced Nurse Practitioner - Palliative Care

About the Role:

We are seeking an experienced Advanced Nurse Practitioner to join our team at Nightingale House Hospice. As an Advanced Nurse Practitioner, you will be responsible for providing high-quality, patient-centered care to individuals with palliative care needs.

Key Responsibilities:

  1. Work autonomously to admit patients to the inpatient unit and provide care for them during their admission for end of life care, symptom management, or respite.
  2. Collaborate with the medical team, nursing team, and consultants to provide comprehensive care for inpatients, outpatients, and patients attending wellbeing or rehab appointments.
  3. Prescribe medicines for patients in accordance with local policies, national standards, and formularies relating to independent prescribing.
  4. Work closely with the nursing team and wider multidisciplinary team to maintain positive working relationships and close teamwork.
  5. Communicate compassionately with patients and their families regarding their condition, treatment, prognosis, and advanced care planning.
  6. Participate in on-call rota and be available to attend to patient needs outside of regular working hours.
  7. Maintain accurate and up-to-date clinical records and complete administrative tasks arising from medical work.
  8. Identify and report issues and incidents that may affect the quality of palliative care services.
  9. Promote and participate in teaching and educational activities to support the development of nursing and medical staff.
  10. Participate in clinical governance activities to ensure the delivery of high-quality, patient-centered care.
  11. Maintain personal development through regular reviews, training, and education to ensure ongoing competence and expertise.
