About the Role

We are seeking a highly skilled and dedicated individual to join our team as a Qualified Practitioner Level One. This role will involve working within a multi-disciplinary service supporting individuals with learning disabilities and care needs.

Key Responsibilities
  • Manage an allocated caseload of clients from across the learning disability service, living in community supported living placements or in their own/family home.
  • Review current care and support plans, identify restrictions, and discuss with stakeholders whether care remains in the individual's best interests.
  • Complete DoLS application paperwork, liaise with providers, and gather necessary documentation.
  • Maintain and update case notes, write reports, and provide evidence in court as required.
  • Work with families to understand the purpose and process of DoLS, support them in understanding the role and responsibilities of the Rule 1.2 representative, and complete referrals to independent advocacy services when necessary.
  • Build effective working relationships with LCC Legal services, work within expected time frames, and understand the legal framework.
About You

We are looking for an individual who holds a Social Work or Nurse qualification, with registration in date with SWE/NMC. The ideal candidate will possess a positive approach, self-motivation, enthusiasm, and resilience. They will also have excellent communication skills, creative thinking, and problem-solving abilities.
