Main ResponsibilitiesThe successful candidate will be responsible for delivering high-quality nursing care to patients with long-term conditions, working autonomously to make clinical decisions and demonstrate critical thinking skills. They will collaborate with the general practice team to meet patient needs, support policy and procedure implementation, and provide leadership where required.
Key Duties- Provide and maintain a high standard of nursing care for patients, including nursing assistance to doctors and other primary healthcare team members.
- Deliver evidence-based practice for patients presenting with long-term conditions and provide preventative health care to the practice population.
- Work collaboratively within the general practice team and wider community teams to meet patient needs, support policy and procedure implementation, and provide leadership where required.

Person SpecificationThe ideal candidate will possess:
- Current registration with the NMC.
- Education to diploma level (A&E or Practice Nurse Qualifications or equivalent).
- Qualified practitioner or completion of a minimum of one recognized level 6 Autonomous Practice Module relating to the diagnosis and management of either injuries or illness.
