We are seeking a skilled Community Psychiatric Nurse to join our team at Service Care Solutions. As a key member of our team, you will be responsible for providing high-quality care and support to individuals experiencing mental health emergencies.
Main Responsibilities- Triage referrals to our service and make recommendations for the best course of action.
- Conduct holistic psychosocial assessments in the community to identify the needs of our service users.
- Complete gatekeeping assessments to determine the suitability of home treatment for individuals who require admission to hospital.
- Attend Mental Health Act assessments and formulate home treatment plans when necessary.
- Implement risk management and care plans to ensure the effective treatment of service users in their own homes.
- Provide support with medication management and concordance in the community.
- Offer telephone support to service users during times of crisis.
- Mental health nursing qualification and registration with the NMC.
- Experience working in a community mental health team.
- Full UK driving licence and DBS clearance.
- Significant experience working in a community setting, with a focus on intensive services and mental health liaison teams.
We offer a competitive hourly rate of £27 and a £250 bonus for starting a contract with us. We also offer a referral bonus of £250 for each successful referral.
