The successful candidate will have experience in palliative care and a strong background in advanced clinical assessment, mentorship, and non-medical prescribing. Excellent communication skills and a patient-centered approach are essential for this role.

Key Responsibilities
  • Plan, implement, and evaluate care delivery according to changing healthcare needs.
  • Identify and address potential palliative care emergencies.
  • Use advanced assessment skills to develop and implement care plans.
  • Provide holistic specialist care for patients, respecting their needs, wishes, and culture.
  • Establish and maintain effective communication with patients, carers, and professionals across health and social services.
