We are seeking an experienced and passionate Adult Nurse Manager to join our team at Leaders In Care Recruitment Ltd. As a key member of our community care team, you will be responsible for providing face-to-face support to adult clients and their families, while creating personalized care plans that meet their unique needs.
Key Responsibilities- Design and deliver tailored care plans that address the physical and physiological needs of clients
- Effectively manage multidisciplinary teams through strong communication and collaboration
- Provide ongoing monitoring, support, and assessment of clients' needs within your caseload
- Deliver nursing interventions, including medication management and care coordination
- Build and maintain strong relationships with clients and their families
- Work closely with community teams, healthcare professionals, and commissioners to establish and maintain bespoke packages of care
- RGN with an active NMC PIN and up-to-date experience of tracheostomy care and ventilators within the last 2 years
- Supportive management style that fosters a positive and productive team environment
- Advanced communication skills, both verbal and written, to effectively liaise with patients, families, and multidisciplinary teams
- Suitable for Band 5 or Band 6 nurses with evidence of effective leadership and management of healthcare assistants
- Full UK driving license desirable but not essential
- Competitive salary up to £45,000 per annum, dependent on experience
- Flexible working arrangements, including 1 day per week working from home and 4 days in the community
- Monday to Friday, 9-5 hours
- 33 days annual leave

- Business travel expenses reimbursed at £0.45 per mile
