Job Summary

We are seeking an experienced Clinical Pharmacist to join our team at Wantage Primary Care Network. As a key member of our pharmacy team, you will play a vital role in providing expert clinical medicines management and supporting the delivery of high-quality patient care.

Main Responsibilities
  • Provide structured medication reviews, including poly-pharmacy and patients in residential care homes, to ensure safe and effective medication use.
  • Manage long-term conditions, medication on transfer of care, and systems for safer prescribing.
  • Work with the pharmacy team on quality improvement and clinical audits, as well as managing aspects of the Investment and Impact Fund (IIF), Quality Outcomes Framework (QOF), and Prescribing Quality Scheme (PQS).
  • Collaborate with the practice teams to ensure compliance with CQC standards and provide education and training on therapeutics and medicines optimisation.
About the Role

This is a patient-facing role in primary care, offering opportunities to utilise your clinical skills to improve the health of the local community. You will work closely with the pharmacy team to develop medicines optimisation services in our practices and contribute to business planning.

Person Specification
  • Master's Degree in Pharmacy (MPharm or equivalent)
  • Specialist knowledge through a Postgraduate Diploma and qualifications, clinical, community, therapeutics, or equivalent
  • Registered with GPhC
  • Minimum of two years' experience as a pharmacist in a hospital, community, or general practice setting
