About the Role

We are seeking a highly skilled Clinical Pharmacist to join our team at Wantage Primary Care Network. As a Clinical Pharmacist, you will play a crucial role in providing expert advice on medicines management and optimisation to improve patient outcomes.

Key Responsibilities

  • Provide structured medication reviews for patients with poly-pharmacy, including those in residential care homes.
  • Manage long-term conditions, including the management of medicines on transfer of care and systems for safer prescribing.
  • Work with the pharmacy team on quality improvement and clinical audits, as well as managing aspects of the Investment and Impact Fund (IIF), Quality Outcomes Framework (QOF), and Prescribing Quality Scheme (PQS).

Requirements

  • Master's Degree in Pharmacy (MPharm) or equivalent.
  • Specialist knowledge through a Postgraduate Diploma and qualifications, clinical, community, therapeutics, or equivalent.
  • Registered with the General Pharmaceutical Council (GPhC).

Desirable

  • Clinical Diploma.
  • Independent prescriber status or a commitment to undertake the course.
  • Knowledge of IT systems, including MS Office and use of EMIS, Docman.
