Community Care Nurse

Sciensus Pharma Services Limited is seeking a highly skilled and compassionate Pediatric Nurse Specialist to join our team. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality nursing care to patients in their own homes.
Key Responsibilities- Assess and Plan Care: Conduct thorough assessments of patients' needs and develop personalized care plans to ensure optimal health outcomes.
- Deliver Care: Provide skilled nursing care to patients in their homes, including administering medications, wound care, and other treatments as prescribed.
- Monitor and Evaluate: Continuously monitor patients' progress and adjust care plans as needed to ensure the best possible outcomes.
- Collaborate with Team: Work closely with other healthcare professionals, including doctors, therapists, and social workers, to ensure comprehensive care and support for patients.
- Document and Communicate: Maintain accurate and detailed records of patient care and communicate effectively with patients, families, and healthcare teams.
- Education and Licensure: Current registration as a Registered Nurse (RGN) with the Nursing and Midwifery Council (NMC) and a valid pediatric nursing qualification.
- Experience: At least 2 years of experience in a pediatric nursing role, with a strong background in community care and home healthcare.
- Skills and Knowledge: Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams. Strong assessment and planning skills, with the ability to develop and implement individualized care plans. Proficiency in using electronic health records and other healthcare software.
