About the Role:
Voluntary Centre Services is excited to announce the recruitment of 2 Social Prescribing Community Link Facilitators to enhance our dedicated Social Prescribing Teams.
Service Overview:
The Social Prescribing Service, developed in collaboration with the Integrated Neighbourhood Working (INW) initiative, aims to empower primary care services to connect patients facing social, emotional, or practical challenges with a variety of local, non-clinical resources.
Key Responsibilities:
As a Social Prescribing Community Link Facilitator, you will play a crucial role in delivering a person-centered approach, assisting individuals in improving their health and well-being by linking them to community services.
Link Facilitators serve as essential community assets, focusing on patient engagement and fostering strong partnerships with healthcare professionals and voluntary/community sector organizations.
Work Environment:
Our organization is committed to flexible working arrangements and values the diverse contributions of part-time staff. We encourage applications for 30-36 hours per week.
Contract Details:
This position is fixed-term until March 2025.
Additional Information:
This role involves regulated activity and necessitates an Enhanced level DBS check.
Annual Leave:
Entitlement of 29 days per year (pro rata).
