About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our Intensive Outreach Team as a Clinical Lead. The successful candidate will be responsible for managing a small caseload of service-users who present with complex mental health needs and will work closely with the team to provide high-quality care and support.

Key Responsibilities
  • Assess, plan, deliver, and evaluate care for service-users referred to the Intensive Outreach Team, including risk assessment and management.
  • Manage a caseload of patients as the named key worker/care coordinator/lead clinician, working collaboratively with service-users and their carers to achieve positive outcomes.
  • Work within a psychosocial/family-centered framework to assess and plan individual packages of care, taking into account the unique needs and circumstances of each service-user.
  • Organize and plan an active clinical caseload as an autonomous practitioner, managing time effectively and liaising closely with other agencies and disciplines.
  • Undertake evaluation of clinical symptoms and disabilities, advising medical staff of any changes requiring psychiatric evaluation and/or intervention.
  • Coordinate CPA (Care Programme Approach) for a caseload of service-users with complex mental health problems, working closely with the Trust's CPA policy.
  • Develop therapeutic relationships with service-users to provide support and assistance, as appropriate, and ensure that the Team is aware of the particular perspective of your professional background in relation to patient care.
  • Participate in mental health promotion/education and provide advice to clients and their carers on maintaining good health and preventing relapse.
  • Administer and monitor the effects and adverse effects of prescribed medication, liaising with medical staff regarding necessary adjustments.
  • Offer advice/information to clients and carers regarding prescribed medications.
  • Ensure the Intensive Outreach Team promotes a positive view of mental health, breaking down prejudice and stigma to facilitate an understanding of problems associated with mental illness.
  • Provide formal clinical supervision to students and other staff within the team.
  • Participate in the team duty rota, taking duty calls as required, and follow the lone working policy for checking staff whereabouts to maintain staff safety.
  • Possess enhanced communication skills to communicate with service-users and carers experiencing a range of mental health experiences, including psychotic symptoms or personality disorder, and who may be in distress.
  • Undertake the role of Lead Clinician for people detained under a Community Treatment Order (Mental Health Act).
  • Complete formal tribunal/hospital managers reports for people detained under a section or a Community Treatment Order.
  • Provide cover for the line manager in their absence.
Person Specification
  • Current professional registration with the appropriate body in healthcare or social work.
  • Evidence of appropriate CPD post-qualifications.
  • Extensive experience in care coordinating people in a community mental health setting.
  • Experience working in the community under an AOT model.
