Intensive Outreach Mental Health Specialist

We are seeking a highly skilled and experienced Mental Health Practitioner to join our Intensive Outreach Team in South East Essex. As a key member of our team, you will work closely with service users who require intensive support and care, and will be responsible for assessing, planning, delivering, and evaluating care to meet their complex needs.
Main Duties and Responsibilities- Assess and manage risk for service users, ensuring their safety and well-being.
- Develop and implement personalized care plans, working collaboratively with service users, their families, and other healthcare professionals.
- Provide high-quality, evidence-based interventions to support service users in achieving their goals and improving their mental health outcomes.
- Work closely with other agencies and disciplines to ensure seamless care coordination and effective communication.
- Participate in mental health promotion and education, providing advice and information to service users and their families on maintaining good mental health and preventing relapse.
- Administer and monitor the effects of prescribed medication, liaising with medical staff as necessary.
- Provide formal clinical supervision to students and other staff within the team.
- Take part in the team duty rota, responding to duty calls and following lone working policies to ensure staff safety.

Person Specification- Current professional registration with a relevant healthcare or social work body.
- Extensive experience in care coordinating people in a community mental health setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with service users, families, and other healthcare professionals.
- Ability to work autonomously, with a high degree of independence and initiative.
- Strong problem-solving and analytical skills, with the ability to think critically and make informed decisions.
