About the Role
  • Work in partnership with other professionals to coordinate and implement multi-agency plans of intervention.
  • Provide early help and deliver evidence-based interventions to support the welfare and safeguarding of children.
  • Lead on assessments and develop support plans for children and families.
  • Act as the lead professional for an allocated caseload, ensuring high standards of case recording and documentation.
  • Engage with children, young people, and their families through persistent outreach and promote their participation in support plans.
Key Responsibilities
  • Work collaboratively with other professionals to achieve positive outcomes for children and families.
  • Develop and implement effective support plans to address the needs of children and families.
  • Provide timely interventions to support the welfare and safeguarding of children.
  • Engage with children, young people, and their families to promote their participation in support plans.
Requirements
  • Experience working in a family support or social care setting.
  • Strong understanding of safeguarding and early intervention practices.
  • Ability to work independently and within a multi-disciplinary team.
  • Excellent communication and case management skills.
  • Experience working with families affected by domestic violence, substance misuse, or disabilities is advantageous.
