Alzheimer Scotland is seeking a highly motivated and compassionate individual to join our team as a Post Diagnostic Support Link Worker in Argyll. As a key member of our Community Dementia Team, you will play a vital role in supporting people with a recent diagnosis of dementia and their families to live well in their community.
Key Responsibilities:
- Provide person-centred support to individuals with dementia and their families, empowering them to make informed decisions about their care and well-being.
- Work collaboratively with healthcare professionals, social workers, and other stakeholders to ensure a seamless and coordinated approach to care.
- Develop and maintain relationships with community groups, organizations, and services to promote awareness and understanding of dementia.
- Utilize digital platforms to facilitate virtual meetings and online support groups, enhancing access to services and promoting social connections.
- Conduct assessments and develop personalized care plans, incorporating the Alzheimer Scotland 5 Pillar Model to support individuals in achieving their goals and aspirations.
- Provide education and training to individuals, families, and caregivers on dementia awareness, management, and support.
- Participate in regular team meetings and contribute to the development of policies and procedures to ensure best practice in dementia care.
Requirements:
- Recognized professional qualification in a relevant field, such as social work, nursing, or psychology.
- Excellent communication and interpersonal skills, with the ability to build rapport with individuals from diverse backgrounds.
- Strong understanding of dementia and its impact on individuals and families, with a commitment to promoting person-centred care.
- Ability to work effectively in a team environment, with a flexible and adaptable approach to meet the needs of service users.
- Enhanced level of the Promoting Excellence Framework or willingness to work towards its completion.
- Full driving license and access to a car, with a willingness to travel within the community.
