About the Role

We are seeking an experienced and motivated Mental Health Coach to join our multidisciplinary team at Norfolk Primary Care C.I.C.

The successful candidate will work across a number of surgeries within our Primary Care Network, providing personalized support to individuals, their families, and carers to empower them to manage their own mental health and live independently.

Key Responsibilities
  • To engage patients in coaching and motivate them through multiple sessions to identify their needs, set goals, and support them to achieve their personalized mental health objectives.
  • To provide interventions such as self-management education and peer support.
  • To support personal choice and positive risk-taking while ensuring that patients understand the accountability of their own actions and decisions, thus encouraging the proactive prevention of further illnesses.
  • To work in partnership with other social prescribing services to connect patients to community-based activities or support charities which enable them to take increased control of their mental health and wellbeing.
