About the Role

We are seeking a dedicated and experienced Mental Health Community Team Leader to join our dynamic team at Derbyshire Healthcare NHS Foundation Trust. In this crucial role, you will provide leadership and support to a team of healthcare professionals, ensuring high-quality care for individuals with severe and enduring mental health needs within the community setting.

Key Responsibilities

  • Lead and supervise a team of nurses and therapists, fostering a collaborative and supportive work environment.
  • Develop and implement care plans tailored to individual patient needs, ensuring person-centered and holistic approaches.
  • Collaborate with multidisciplinary teams, including GPs, social workers, and psychiatrists, to provide comprehensive care coordination.
  • Manage complex cases involving individuals with challenging behaviors, utilizing effective de-escalation techniques and crisis intervention strategies.
  • Promote evidence-based practice and continuous professional development within the team.

Essential Criteria

  • Registered Nurse or Occupational Therapist with a relevant degree/diploma.
  • Proven experience working as a Band 6 or equivalent in a mental health community setting.
  • Demonstrated expertise in managing complex and sensitive information, utilizing strong motivational and negotiation skills.
  • Defined management/leadership skills with the ability to motivate and influence staff effectively.
  • Knowledge of management structures and processes, coupled with the ability to work autonomously and set priorities.
