We are seeking a skilled Long Term Condition Nurse to join our team at Cumbria Health. About the RoleThis is a 16-hour per week, 2-day per week position (Monday and Wednesday) that requires a strong clinical background in long-term condition management. You will be working in a dynamic and supportive environment, with opportunities for professional development and growth.
Key Responsibilities- Deliver high-quality care to patients with long-term conditions, including asthma, COPD, diabetes, and chronic heart disease.
- Manage programmes of care for patients with long-term conditions, planning, providing, and evaluating care under agreed local and national guidelines.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in clinical meetings and contribute to the development of clinical guidelines and protocols.
- Maintain accurate and concise documentation, adhering to local and national standards.
- Current NMC registration and 1st Level registration.
- Relevant long-term condition management qualification or experience.
- Evidence of recent Continual Professional Development.
- Cytology and relevant Vaccination and Immunisation training.
