We are seeking a highly skilled and experienced Registered Mental Health Nurse to join our Integrated Care Homes Service as a Specialist Dementia Outreach Nurse. This is an exciting opportunity to work with a dedicated team to provide high-quality care and support to individuals with dementia living in care homes.
Key Responsibilities- Provide comprehensive healthcare input for clients referred to the service, including prioritization, assessment, planning, advice, treatment, evaluation, and discharge.
- Accept clinical responsibility for a designated caseload of clients and organize this effectively with regard to clinical priorities and use of time.
- Delegate and supervise duties to unqualified members of staff using agreed protocols.
- Undertake initial assessments and care-coordinator responsibilities as part of the Outreach Team.
- Monitor and review medication where appropriate.
- Case manage clients in receipt of fully funded continuing healthcare funded by Nottingham City CCG.
- Support patients on long-term discharge to residential/nursing homes and support clients/families through this transition.
- Determine by assessment and liaison with other relevant agencies the advice, education, and treatment requirements of each client.
- Provide highly specialized advice, teaching, and training to other members of the Integrated Team regarding the management of clients with problems related to dementia.
- Train, supervise, and manage more junior staff, support staff, and students.
- Provide education/teaching to residential and nursing homes relating to dementia care across a range of healthcare needs.
- Gain valid informed consent and identify through assessment, clients who lack capacity, and work within a legal framework with clients who lack the capacity to consent to treatment.
- Work as a lone practitioner with supervision from the clinical lead.
- Experience of working with patients with complex dementia needs.
- Experience of team working and caseload management.
- Evidence of good interpersonal and leadership skills.
- Ability to work within a culturally diverse population.
As a Specialist Dementia Outreach Nurse with Nottingham CityCare Partnership CIC, you will have the opportunity to work with a dedicated team to provide high-quality care and support to individuals with dementia living in care homes. You will be responsible for providing comprehensive healthcare input, accepting clinical responsibility for a designated caseload of clients, and delegating and supervising duties to unqualified members of staff.
