About the Role:
- As a Qualified Social Worker, you will work collaboratively with service users and their families in a recovery-oriented model, promoting independence, choice, wellbeing, and social inclusion.
- You will contribute to the delivery of mental health services and provide a casework service in collaboration with service users and CMHT colleagues.
- You will practice in accordance with CPA and recovery model of care, managing a caseload including assessment, planning, implementing and evaluating care, negotiating for and coordinating the application of resources and seeking modification of attitudes or behaviour when required.
Responsibilities:
- Develop and implement individualized care plans that promote the well-being and independence of service users.
- Conduct thorough assessments and evaluations to identify the needs of service users and develop effective interventions.
- Collaborate with multidisciplinary teams to provide comprehensive care and support services.
- Provide education and support to service users and their families to promote self-advocacy and empowerment.
Requirements:
- Social Work Degree or equivalent
- Social Work England registration
- Enhanced DBS
- Office based
