We are looking for a Senior Clinical Practitioner to become a vital part of our stroke care team, which includes General Practitioners with a focus on stroke and frailty, as well as Advanced Practitioners. Our unit is also supported by a visiting Stroke Consultant.
The facility comprises 30 beds located within a community hospital, providing a setting where patients who are medically stable can undergo rehabilitation assessments. Our multidisciplinary team collaborates effectively to ensure safe and efficient care for patients as they recover from hospital stays and prepare to return home.
We offer a nurturing environment dedicated to your continuous professional development, education, and further training. You will have the opportunity to apply your expertise in a supportive atmosphere aimed at enhancing your skills in urgent care, stroke management, and other chronic conditions.
Key Responsibilities
As a member of our stroke medical team, you will be primarily responsible for the clinical management of our 30 inpatient individuals.
Patients are admitted for rehabilitation and recovery following acute care for stroke. Throughout their stay, they will require medical interventions to:
- Manage their stroke effectively.
- Optimize their medical condition to enhance rehabilitation potential.
- Collaborate with the multidisciplinary team to improve overall health and stroke management.
- Provide urgent medical responses to any new health deteriorations.
You will oversee the ongoing management of patients' stroke admissions and any co-existing health issues, supporting the broader multidisciplinary team in delivering safe and effective rehabilitation care to facilitate recovery and home transition.
- Conduct assessments and initial evaluations of newly admitted patients.
- Prepare medication charts to ensure safe medication administration.
- Document and address significant medical events from patients' admissions.
- Work collaboratively with colleagues and pharmacists to ensure secondary stroke prevention measures are implemented.

Sirona care & health CIC is committed to recruiting overseas nursing and therapy professionals through an NHS Trust Supplier, adhering to the NHSE Code of Practice regarding applicants from Red List countries. Therefore, we cannot accept direct applications from candidates not currently residing in the UK.
This position is not classified as an Advanced Practice role aligned with the HEE Multi-professional Framework for Advancing Practice. These roles are currently under review and may undergo a name change in the near future. This is due to the requirement for candidates to work across all four pillars of Advanced Practice to MSc level, which is not applicable for this position, as it is banded at AFC Band 7. However, it does necessitate a skill set that operates at or towards MSc level in the Clinical Pillar of Advanced Practice. This role will not qualify for training funding for a full MSc in Advanced Practice, although training may be supported on an individual basis, depending on service and personal learning needs, following the Sirona CPD funding application processes. Qualifications and Experience
Essential
- Advanced skills and experience in managing patients with chronic conditions and urgent care needs.
- Proficiency in clinical examination techniques.
- Experience in developing professional and management practices within your area and across related disciplines relevant to the service.
Desirable
- Experience working at an Advanced Practitioner level in a healthcare environment.
Qualifications
Essential
- Current registration with NMC or HCPC.
- Independent Prescribing Non-Medical Prescribing Qualification.
- Evidence of post-qualification modules relevant to core areas of Advanced Practice.
- Proof of ongoing professional development.
Desirable
- Recognized postgraduate management or leadership qualification, or a willingness to pursue one.
- Master's level 7 in Advanced Practice or completion of an Advanced Practitioner Pathway.
