About the Role

We are seeking an experienced Advanced Nurse Practitioner to join our team at The Highlands Practice. As an Advanced Nurse Practitioner, you will provide high-quality patient care, working autonomously and as part of a multidisciplinary team.

Key Responsibilities

  • Provide same-day care for patients, including telephone triage, initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.
  • Work as an autonomous practitioner, using your clinical judgment to diagnose, treat, or refer patients.
  • Diagnose and manage both acute and chronic conditions, integrating drug- and nondrug-based treatment methods into a management plan.
  • Make direct referrals to primary, secondary, and social services within locally agreed pathways, guidance, and protocols.
  • Work with patients and their carers to support compliance with and adherence to prescribed treatments.
  • Ensure care plans are in place, including emergency health care plans following discussion with the patient and family members.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, within scope of practice.
  • Ensure complete and accurate documentation of each and every patient contact.
  • Work as an autonomous practitioner and as part of multi-disciplinary and multiagency teams in order to ensure patients needs are met.
  • Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.

Professional Requirements

  • Adhere to the NMC Code of Professional Conduct and always work within the scope of professional practice.
  • Ensure that professional practice adheres to organisational and NMC policies, procedures, and guidelines.
  • Maintain a professional manner and act as a positive role model to other members of the Primary Care team.
  • Maintain confidentiality with regard to information pertaining to patients and staff.
  • Ensure that the service interfaces with all other departments in a professional and productive manner, providing an effective service to partner organisations and other service providers.
  • Where required provide supervision and mentoring to other staff in order to help them develop their knowledge, skills and abilities.
  • Deliver care according to NICE guidelines and evidence-based care.
  • Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities.

Person Specification

We are looking for an experienced Advanced Nurse Practitioner with a strong clinical background and excellent communication skills. The ideal candidate will have:

  • Previous experience of working in primary care.
  • Experience of working in the role and not working towards a qualification.
  • An active registration with the Nursing and Midwifery Council.
  • 2-3 years of clinical experience.
  • An MSc nursing degree.
