We are seeking an experienced and skilled Advanced Nurse Practitioner to join our Complex Care Team at Meon Health Practice.
About the RoleThis is an exciting opportunity to work in a dynamic and growing team, delivering high-quality care to patients in their own homes and in residential and nursing homes.
As an Advanced Nurse Practitioner, you will be responsible for providing autonomous clinical care to patients, working closely with other healthcare professionals to ensure seamless pathways of care.
You will have the opportunity to develop and implement new patient services, participate in quality improvement initiatives, and contribute to the assessment of community health needs.
Key Responsibilities- Provide proactive consultations with patients in their own homes and in care homes, using advanced autonomous clinical skills and a broad theoretical knowledge base.
- Work alongside Clinical Pharmacists to deliver medicines-related care and ensure that professional standards are maintained.
- Contribute to the development of patient and family-centred strategies to address community health needs.
- Participate in quality assurance strategies, including audit and peer review, to monitor the effectiveness of your clinical practice.
- Record accurate and timely consultation data within patient records.
We are looking for a registered Nurse with post-graduate qualifications relevant to the position, who has a strong desire to achieve the best possible outcomes for patients in the community setting.
The ideal candidate will have a good understanding of how Primary Care works and be motivated to help develop the services as the team evolves to meet the needs of the population.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
