Clinical Lead

We are seeking an experienced Community Registered Nurse to lead our forward-thinking Community Nursing Localities team in Portsmouth. As a Clinical Lead, you will be responsible for managing and coordinating a Nursing Team to provide high-quality, person-centered care to patients with multiple complex long-term conditions.
Main Responsibilities- Diagnose, prescribe, and implement care interventions for patients on your caseload, focusing on avoiding unnecessary hospital admissions.
- Plan and coordinate care interventions for patients with multiple, complex long-term conditions, working collaboratively with other healthcare professionals.
- Ensure integration of all elements of care, increase patient choice, and prevent unnecessary admissions to hospital while reducing the length of necessary hospital stays.
- Registered Nurse with a District Nurse Specialist Qualification or other relevant qualifications.
- Teaching certificate/module and degree level or equivalent.
- Experience in planning, delivering, and evaluating care to address people's changing health needs.
- Able to develop and sustain relationships across professional boundaries.
