Job Overview

We are seeking a highly skilled and experienced Advanced Nurse Practitioner to join our team at Holmes Chapel Health Centre.

Key Responsibilities
  1. Provide high-quality patient care in a clinical environment, utilizing your expertise to deliver consultations and assessments.
  2. Develop and maintain accurate patient records, adhering to legislation, policies, and procedures.
  3. Implement evidence-based practice, adhering to NSF and NICE guidelines to ensure optimal patient outcomes.
  4. Contribute to quality governance systems, participating in self and peer review, benchmarking, and formal evaluation to drive continuous improvement.
  5. Collaborate with the wider team to support colleagues in meeting patient needs, promoting a culture of teamwork and shared learning.
  6. Participate in the management of patient complaints, clinical incidents, and near-miss events, utilizing a structured framework to identify learning opportunities.
  7. Apply legal policies to identify and support vulnerable patients, adhering to statutory child and adult health procedures and local guidance.
  8. Manage risk within your areas of responsibility, ensuring adequate measures are in place to protect staff and patients.
  9. Support local projects as agreed with the practice management team, contributing to the development of our services.
  10. Undertake mandatory and statutory training to maintain your professional competence.
  11. Apply infection control measures according to local and national guidelines, promoting a safe working environment.
  12. Utilize technology to support management, planning, and monitoring, presenting and communicating information effectively.
  13. Review and process data using templates and accurate SNOMED/Read codes to facilitate easy retrieval for monitoring and audit purposes.
  14. Model good practice in equality and diversity, respecting patients' rights to choose their care providers and participate in care.
  15. Assist marginalized groups to access quality care, promoting inclusivity and respect.
