We are seeking an experienced Advanced Clinical Nurse Specialist to join our Marie Curie REACT project in Bradford. The successful candidate will be responsible for providing specialist palliative care nursing services to patients with complex needs, working as part of a multidisciplinary team.
Key Responsibilities:
- Provide specialist palliative care nursing services to patients with complex needs
- Work as part of a multidisciplinary team to deliver high-quality patient care
- Develop and implement individualized care plans to meet the needs of patients and their families
- Collaborate with other healthcare professionals to ensure seamless care transitions
- Contribute to the development and implementation of quality improvement initiatives
Requirements:
- Registered Nurse on NMC register (Adult)
- Relevant Nursing Degree
- Mentorship/Teaching qualification
- Postgraduate Diploma in Health-related topic
- Working towards a Master of Nursing/Palliative care
- Non-Medical prescribing qualification
- A full driving licence with access to a vehicle insured for business use and a willingness to travel
