Townships 2 PCN is seeking a dedicated and compassionate Occupational Therapist to join our growing team. This role focuses on providing specialized care to older adults experiencing moderate to severe frailty, aiming to enhance their independence, safety, and overall well-being within their homes.

As an Occupational Therapist, you will play a vital role in assessing patients' needs, developing personalized care plans, and delivering interventions that promote functional improvement. You will collaborate closely with other healthcare professionals, including GPs, social prescribers, and district nurses, to ensure comprehensive and coordinated care.

Key Responsibilities:

  • Conduct thorough assessments of patients' physical, cognitive, and psychosocial needs related to frailty.
  • Develop and implement individualized treatment plans to address identified challenges and promote functional independence.
  • Provide hands-on therapy interventions, including adaptive equipment recommendations, home safety modifications, and strategies for daily living activities.
  • Educate patients and their families on self-management techniques, fall prevention strategies, and community resources.
  • Collaborate effectively with the multidisciplinary team to ensure seamless care transitions and optimal patient outcomes.
  • Maintain accurate documentation of assessments, interventions, and progress notes.

Skills and Qualifications:

  • Registered Occupational Therapist with current licensure.
  • Experience working with older adults experiencing frailty or other geriatric conditions.
  • Strong assessment, planning, and intervention skills.
  • Excellent communication, interpersonal, and teamwork abilities.
  • Commitment to providing patient-centered care.
