Position Overview
Important Note: This role is not part of a Mental Health Team
The Community Treatment Team (CTT) is a recognized hospital avoidance initiative aimed at alleviating the pressures faced by Emergency Departments nationwide.
As a vital member of the CTT, the practitioner will be entrusted with the ongoing assessment and management of patients, employing advanced skills and specialized knowledge to evaluate, plan, implement, and review care strategies.
The role includes facilitating referrals to various community services, ensuring safe patient management throughout their CTT care episode.
CTT collaborates closely with the London Ambulance Service, responding to emergency calls. Following comprehensive training, practitioners will have the opportunity to engage with the CTT/LAS vehicle to further support service objectives.
Key Responsibilities:
- Manage a specialized caseload of clients across A&E, NHS 111, Frailty units, and community settings, utilizing the nursing process for assessment, planning, implementation, and evaluation.
- Provide professional and clinical guidance to team members and collaborative partners, including Acute hospital trusts, GPs, NHS 111, and Ambulance Services.
- Contribute to the establishment of a robust clinical governance framework for high-quality service delivery.
- Assist in the daily management of the CTT, coordinating workloads and team development.
- Engage in the implementation of service advancements impacting Community Services.
- Foster effective communication and relationships across all disciplines and services.
- Function as an autonomous practitioner to assess, plan, implement, review, and evaluate care, including complex assessments.
- Ensure high-quality service delivery, providing care throughout the year, with flexible working options available.
About NELFT
Probationary Period:
This position is subject to a probationary period, with internal applicants exempt unless currently undergoing a probationary evaluation.
Onboarding with NELFT:
We prioritize a comprehensive onboarding experience for new employees, who will join the Trust on the first Monday of each month. This includes an extensive induction program lasting up to two weeks, covering mandatory training, systems training, and equipment allocation. New starters will also have opportunities to meet senior management and participate in sessions focused on engagement and well-being.
High-Cost Area Supplement:
This position qualifies for a High-Cost Area Supplement of 15% of the basic salary, with minimum and maximum payments specified.

Job Responsibilities:
The practitioner will receive support to develop skills necessary for assessing acutely unwell adults, ensuring their treatment aligns with best practices.
The successful candidate will work collaboratively within the Emergency Department (ED), assessing patients to determine their care needs during crises.
Utilizing evidence-based practices, the candidate will conduct thorough assessments to facilitate timely and effective patient discharges. Engagement with multidisciplinary team members, including Occupational Therapists, Physiotherapists, Health Care Assistants, Nurses, Doctors, Pharmacists, and Social Workers, is essential.
The band 6 practitioner will supervise junior staff and will be supported in developing skills to take on senior responsibilities in their absence. Opportunities for clinical educator training and student support will be provided.
Clinical Skills Required:
- Conduct comprehensive assessments of a defined client group, planning and implementing client-centered interventions while evaluating treatment outcomes.
- Perform holistic patient assessments, including wound care, medication administration, injections, phlebotomy, IV cannulation, and clinical observations.
- Execute thorough triage processes, identifying urgent cases requiring immediate medical attention.
- Utilize patient medical and social histories as part of the nursing assessment process.
- Promote evidence-based practices and participate in clinical trials and audits within the CTT.
- Delegate clinical and administrative tasks effectively to support staff while monitoring outcomes.
- Maintain accurate and timely records of all treatment processes and patient interactions.
- Serve as a resource for team members, supporting quality assurance in all actions.
- Engage in clinical meetings and case conferences, presenting detailed professional insights.
- Understand the impact of disabilities on clients, patients, caregivers, and the broader community.
- Account for professional actions in accordance with Nursing & Midwifery standards or HCPC guidelines.
- Encourage evidence-based practices within the service.
- Manage referrals and caseloads under the guidance of the team leader, ensuring appropriateness and compliance with established criteria.
