In this role, you will provide high-quality, person-centered care to individuals with learning disabilities within a community setting. Your expertise will be instrumental in supporting their well-being, promoting independence, and empowering them to live fulfilling lives.
Responsibilities:- Conduct comprehensive assessments of service users' needs, including risk identification and management.
- Develop and implement individualized care plans based on evidence-based practices.
- Administer medications and provide wound care as required.
- Collaborate effectively with multidisciplinary teams, including doctors, social workers, and therapists.
- Advocate for service users' rights and ensure their voices are heard.
- Provide education and support to families and caregivers.
The ideal candidate will possess:
- Registration as a Registered Nurse Learning Disabilities (RNLD) with the NMC.
- Substantial post-registration experience in learning disabilities or related areas, including community settings.
- A strong understanding of risk assessment and management principles.
- Excellent communication, interpersonal, and problem-solving skills.
- A commitment to providing person-centered care and promoting service users' well-being.
