About the Role

We are seeking a highly skilled and dedicated individual to join our team as a Community Connector – Mental Health. This exciting opportunity is part of a new transformation fund to pilot innovative models of primary and community mental health care.

Key Responsibilities
  • Work with a range of professionals across Primary Care Networks (PCNs) to develop and mobilise integrated primary care mental health services.
  • Provide early intervention and promote early triage, assessment, and formulation of need and care plans for patients with mental health presentations.
  • Coordinate and support patients to access a wide range of community services and resources, including social care, housing, family debt, and employment counselling.
Requirements
  • A Diploma or degree in a relevant subject, such as Counselling, Psychology, Mental Health, Social Work, or Health and Social Care.
  • Good communication skills, both written and verbal.
  • Proficiency in Microsoft Word and Excel.
  • Ability to travel around the area required for the role, with own transport preferred.
