Position Overview

We are seeking a full-time (hours per week, Monday - Friday) Band 4 Rehabilitation Support Practitioner to become a vital member of our dedicated Acute Medicine Therapy Team.

Main Responsibilities

The responsibilities of this role include:

  • Assisting patients with exercise routines and mobility enhancement
  • Supporting therapists in planning for patient discharge
  • Carrying out functional tasks to aid patients in regaining their previous level of independence
  • Performing administrative tasks
  • Conducting home visits to evaluate patients' discharge requirements
  • Assessing, practicing, and providing necessary equipment.

Qualifications

Essential

  • NVQ III in Diagnostics & Therapeutics or a relevant qualification
  • Evidence of relevant short courses and in-service training completed.
  • English and Maths GCSE grade 4 or functional skills level 2

Experience

Essential

  • A minimum of 2 years in a Therapy support worker capacity
  • Experience with IT systems
  • Demonstrated significant relevant experience working with patients in an acute care environment.

Desirable

  • Experience working with patients in a group setting

Knowledge

Essential

  • Understanding of the roles of Occupational Therapy and Physiotherapy in acute healthcare settings.
  • Knowledge of long-term health conditions

Skills

Essential

  • Excellent written communication skills
  • Ability to communicate effectively through verbal and non-verbal means, including active listening
  • Confidence and competence in patient handling tasks, ensuring safe practices with equipment
  • Capability to undertake specialized treatment programs/techniques independently.
