Job Summary

We are seeking an experienced Practice Nurse to join our team. The successful candidate will be responsible for implementing processes for the effective management of patients with long-term conditions, including chronic disease management, health promotion, and well-being programs.

Main Responsibilities
  • Act as support to the Infection Prevention Control (IPC) lead and work to the IPC Policy.
  • Develop, implement, and embed health promotion and well-being programs.
  • Implement and evaluate individual treatment plans for chronic disease patients.
  • Identify, manage, and support patients at risk of developing long-term conditions.
  • Provide routine nursing care to patients as required.
  • Diagnose and manage acute and chronic conditions.
  • Prescribe medication for patients in accordance with PGDs and own scope of practice.
  • Review patient medications to enhance compliance.
  • Provide guidance to patients on the use of prescribed and over-the-counter medications.
  • Oversee the provision of wound care services to patients.
  • Coordinate the provision of travel medicine services.
  • Request pathology services as necessary.
  • Process pathology results as required.
  • Oversee the provision of chronic disease clinics.
  • Maintain accurate clinical records.
  • Ensure read codes are used effectively by all members of the nursing team.
  • Ensure the appropriate maintenance and use of chronic disease registers.
  • Develop, implement, embed, and participate in well-woman and well-man clinics.
  • Chaperone patients where necessary.
  • Assist GPs with minor surgery when required.
  • Prioritize health issues and intervene appropriately.
  • Support the team in dealing with clinical emergencies.
  • Recognize, assess, and refer patients presenting with mental health needs.
  • Implement vaccination programs for adults and children.
  • Support patients in the use of their prescribed medicines or over-the-counter medicines.
  • Liaise with external services/agencies to ensure patient support.
  • Delegate clinical responsibilities appropriately.
  • Support the clinical team with all safeguarding matters.
  • Understand practice and local policies for substance abuse and addictive behavior.
  • Deliver opportunistic health promotion where appropriate.
Secondary Responsibilities
  • Develop and review audit protocols and processes for the practice.
  • Develop, review, and implement nursing protocols in conjunction with partners.
  • Support all members of the nursing team, providing guidance when necessary.
  • Participate in local initiatives to enhance service delivery and patient care.
  • Support and participate in shared learning within the practice.
  • Continually review clinical practices, responding to national policies and initiatives.
  • Participate in the review of significant and near-miss events.
  • Coordinate the nursing team rota.
  • Lead the planning and implementation of changes within the nursing team.
  • Support the practice team with the reviewing and implementation of practice policies and protocols.
Person Specification

Qualifications

  • Registered nurse.

Desirable

  • Postgraduate diploma or degree in Chronic Disease Management.
  • Nurse prescriber.
  • Diabetes Certificate.
  • Spirometry Certificate.

Experience

  • Experience of working as a practice nurse or community nurse.
  • Experience of working in a primary care environment.
  • Experience of chronic disease management.
  • Experience of working autonomously.

  • Wound care/removal of sutures and staples.
  • ECGs.
  • Venepuncture.
  • New patient medicals.
  • Chaperone procedure.
  • Requesting pathology tests and processing the results.
  • Travel medicine.
  • Hypertension.
  • Asthma.
  • CHD.
  • Immunisations.
  • Women's health.
  • Understand the importance of evidence-based practice.
  • Broad knowledge of clinical governance.
  • Ability to record accurate clinical notes.
