Bosworth Medical Centre is seeking an experienced Practice Nurse to join our team. As a key member of our clinical team, you will be responsible for delivering high-quality patient care and contributing to the effective management of chronic diseases.
Main Responsibilities- Implement evidence-based practice for the management of patients with long-term conditions, including chronic disease management, health promotion, and well-being programs.
- Work collaboratively with the multidisciplinary team to ensure nursing services are delivered effectively to the entitled patient population.
- Develop and implement health promotion and well-being programs, including chronic disease clinics and travel medicine services.
- Provide routine nursing care to patients, including wound care, ECGs, venepuncture, and new patient medicals.
- Diagnose and manage acute and chronic conditions, including hypertension, asthma, CHD, and immunisations.
- Prescribe medication for patients in accordance with PGDs and own scope of practice.
- Review patient medications to enhance compliance and provide guidance on the use of prescribed and over-the-counter medications.

Person Specification- Registered nurse with experience of working as a practice nurse or community nurse.
- Experience of working in a primary care environment and chronic disease management.
- Postgraduate diploma or degree in Chronic Disease Management or a related field (desirable).
- Nurse prescriber qualification (desirable).
- Diabetes and COPD certification (desirable).
