We are seeking a highly skilled and experienced Clinical Lead to join our team at North East London Foundation Trust. As a key member of our community health team, you will be responsible for delivering high-quality patient care and leading the development of services surrounding long-term conditions.

The successful candidate will have a strong background in community health, with experience in managing complex caseloads and working collaboratively with GPs, social services, and other healthcare professionals. You will be a registered nurse with at least 5 years of experience, and have a minimum of 2 years of experience in a leadership role.

Main Responsibilities:
  • Manage a caseload of patients with complex health and social care needs
  • Develop and implement strategies for improving patient outcomes and service delivery
  • Work collaboratively with the multidisciplinary team to deliver high-quality patient care
  • Provide clinical supervision and support to colleagues
  • Contribute to the development of policies and procedures related to community health
Requirements:
  • Registered nurse with at least 5 years of experience
  • Minimum of 2 years of experience in a leadership role
  • Experience in managing complex caseloads and working collaboratively with GPs, social services, and other healthcare professionals
  • Strong background in community health and long-term conditions
  • Ability to work autonomously and as part of a team
