The Role
As a Hospital Support Worker, your daily tasks will be diverse and fulfilling. Here’s a glimpse of your responsibilities:

  • Provide support to family members and unpaid caregivers when they or the individuals they care for are discharged from medical facilities;
  • Conduct initial discussions with caregivers to guide them through our services;
  • Perform initial evaluations with caregivers to identify their needs arising from their caregiving roles, and collaboratively work with them using a strengths-based approach;
  • Deliver one-on-one wellbeing support for caregivers for up to six weeks following discharge;
  • Work proactively to prevent hospital admissions by supporting caregivers.

The Person
Required experience/qualifications:

  • Experience in providing one-on-one guidance or support within the health and social care sector or a relevant field;
  • Understanding of caregivers and their needs as outlined in the Care Act;
  • Strong communication and interpersonal skills, with the ability to empathize with caregivers' needs;
  • Proven experience in assessing and addressing clients' needs (including emotional support) through short-term intervention planning;
  • Basic proficiency in using computers, databases, and relevant software;
  • Commitment to a caregiver-led and caregiver-focused approach, ensuring high-quality services;
  • Excellent organizational, administrative, and record-keeping skills, with the ability to write clearly and professionally;
  • Valid driver’s license or ability to meet travel requirements associated with the role;
  • Right to work in the UK.

A foundational understanding of the Care Act 2014 is preferred, but a willingness to learn is essential.

Do I need experience in the adult social care sector?

Ideally, yes, but if you meet most of the above criteria, we will provide the necessary training and resources to help you excel in this role.
