The Role
As a Healthcare Support Specialist, your daily responsibilities will be diverse and engaging. Here is a glimpse of what your role will encompass:

  • Assisting family members and unpaid carers when they or the individuals they care for have been discharged from medical facilities;
  • Conducting initial discussions with carers to navigate their journey through our services;
  • Performing initial evaluations with carers to identify needs arising from their caregiving roles, utilizing a strengths-based approach to address these needs;
  • Providing one-on-one wellbeing support for carers for up to six weeks following hospital discharge;
  • Working proactively to prevent hospital admissions by supporting carers.

The Person
Previous experience/qualifications:

  • Experience in delivering one-on-one advice or support within the Health and Social Care sector or a relevant role;
  • Demonstrated understanding of carers and their needs under the Care Act;
  • Excellent communication and interpersonal skills, with the ability to empathize with the needs of carers;
  • Proven experience in assessing and addressing client needs (including emotional) through support planning for short-term interventions;
  • Basic proficiency in using computers, databases, and relevant software;
  • Commitment to working in a carer-led and carer-focused manner, ensuring high-quality services;
  • Strong administration, organizational, and record-keeping skills, with the ability to write clearly and professionally;
  • Valid driving license or ability to meet travel requirements associated with the role;
  • Right to work in the UK.

A foundational understanding of the Care Act 2014 is preferred, but a willingness to learn and comprehend the Act is essential.
