Job Title: Adults Social Work Advanced Practitioner

We are seeking an experienced and skilled Adults Social Work Advanced Practitioner to join our team at Somerset Council.

About the Role

This is an exciting opportunity to work in a dynamic and supportive environment, overseeing a team of Social Workers and providing leadership and guidance to ensure the best possible outcomes for individuals and their families.

Key Responsibilities
  • Manage and lead a team of Social Workers, providing guidance and support to ensure the delivery of high-quality services.
  • Work closely with health and community partners to develop and implement effective solutions to support individuals and their families.
  • Use performance and hospital-related data to inform decision-making and resource allocation.
  • Collaborate with key partners to facilitate improved provision of services, team training, and recruitment.
  • Guide your team to support and empower residents of Somerset to remain independent for longer in their communities.
