Key Responsibilities- Leadership and Management: Provide leadership and guidance to the nursing and care teams, ensuring that they are equipped to deliver high-quality care to our service users.
- Care Planning and Delivery: Develop and implement individualized care plans in partnership with service users, families, and other healthcare professionals, ensuring that goals are patient-focused, realistic, and achievable.
- Quality and Compliance: Ensure that all care services are delivered in accordance with statutory, mandatory, and contractual requirements, and that quality standards are maintained and exceeded.
- Collaboration and Communication: Work collaboratively with other healthcare professionals, including case managers, integrated care systems, general practitioners, consultants, and other healthcare professionals within the multidisciplinary team.
- Professional Development: Maintain and develop your own professional skills and knowledge, and support the professional development of the nursing and care teams.
- Professional Qualifications: Registered Nurse with no restrictions.
- Experience: Proven experience of working in community care, with a strong understanding of the needs of individuals with brain injuries, spinal injuries, and cerebral palsy.
- Skills: Excellent communication and interpersonal skills, with the ability to work effectively with service users, families, and other healthcare professionals.
- Knowledge: Strong knowledge of care planning, delivery, and management, with a focus on quality and compliance.
- Personal Qualities: A strong commitment to quality standards, with a passion for delivering outstanding care in a rewarding and sometimes emotionally challenging environment.
