Position Overview
Are you seeking a rewarding opportunity? Our Integrated Network Team is in need of a proactive individual to assist and evaluate patients post-hospital discharge, enabling them to continue living independently in their homes.
The role involves delivering safe, patient-focused, effective, and evidence-informed clinical services within the community.
You will collaborate within the Adult Service to provide a comprehensive array of clinical interventions, both acute and chronic, supported by a solid competency framework for adults in their residences and other environments.
As part of a cohesive multi-disciplinary team (MDT), you will work alongside Nurses, Physiotherapists, Occupational Therapists, and other Allied Health Professionals.
This position requires flexibility in working days and shifts across a 7-day period to accommodate service demands.
Key Responsibilities
- Conduct thorough holistic assessments of service users.
- Formulate and implement individualized care plans.
- Assist with the coordination hub within the locality.
- Supervise junior staff and contribute to their professional development through mentoring, one-on-ones, and training.
- Complete necessary training, supported by a comprehensive competency framework, within the designated timeframe to fulfill the role's requirements and deliver a competent clinical service.
- Liaise with the senior leadership team in the locality to ensure seamless service delivery.
- Identify potential risks and take appropriate actions to mitigate risks to yourself, service users, and colleagues in accordance with organizational policies and procedures.
- Organize shifts and assign care responsibilities to the team in line with policy.
- Serve as a point of escalation for staff to report any issues or patient deteriorations.
- Pursue additional training as necessary to develop advanced skills and enhance patient care.

Qualifications
Essential
- BSc or equivalent experience in Paramedicine, Nursing, Occupational Therapy, or Physiotherapy.
- Current relevant professional registration.
Desirable
- Up-to-date knowledge of Evidence-Based Practice, local and national standards.

- Strong communication skills - written, electronic, verbal, and non-verbal.
- Ability to effectively lead a team with senior support when necessary.
- Capacity to make clinically sound judgments when addressing a variety of conditions.

Desirable
- Recent relevant experience in a community setting, including management of complex frail patients.
- Experience collaborating with a diverse range of professionals in Health or Social Care.
