We are seeking dedicated professionals with a strong foundation in pre-hospital emergency medicine to join our welcoming and clinically vibrant team, committed to providing care to the expanding population of the Highlands. Due to a team member reducing their hours, NHS Highland is looking to recruit an individual on a part-time permanent basis to fulfill the role of 'PICT Physician' within the collaborative Doctor/Advanced Practitioner PICT response team operating from the Emergency Department. This team has been delivering emergency medical services to the Highlands for several years.

The Advanced Practitioner role within the team is undergoing ongoing training, development, and investment from both SAS and NHSH, featuring a mix of fully qualified and trainee personnel contributing to the team roster. Their training and clinical governance are governed by a mutual agreement between SAS and NHSH.

Candidates should possess core experience in various clinical roles, including both primary and secondary care. The case mix encompasses a wide range of emergency situations, particularly focusing on major trauma, cardiac arrest, pediatric emergencies, and multiple casualty incidents.

The responsibilities of a physician providing 'ED Outreach' in our often remote and rural communities are both academically and physically demanding. The senior decision-maker role of the PICT physician involves a high level of clinical risk management for emergency patients at the scene, including radio and phone triage of cases for attendance, and direct professional support for ambulance personnel attending patients en route to the Trauma Unit. While the primary focus of the role is on prehospital attendance, the team also frequently provides medical escort for the transfer of critically ill patients to the hospital and delivers clinical interventions within the Trauma Unit ED that do not hinder their immediate availability for prehospital tasks. Therefore, we seek physicians with the necessary professional seniority, prehospital training, and experience to act as the senior decision-maker within a team of advanced generalists, ideally with experience in doctor/advanced practitioner response teams.

Suitable candidates will have previously trained and worked at the level expected of pre-hospital physicians working for the Scottish Ambulance Service, as outlined in the existing BASICS Scotland / SAS Memorandum of Understanding. Proficiency in a broad range of clinical interventions is a prerequisite for application and a minimum standard prior to approval for team participation. As prehospital emergency medicine continues to evolve, it is anticipated that the treatments and procedures employed by the team will also advance over time, with appropriate training and governance established before clinical implementation. A robust clinical governance framework is in place, featuring direct case review and analysis during regular team meetings. We seek physicians who are eager to learn as well as to teach.

It is expected that the successful applicant will undertake this role as a relatively minor time commitment compared to their primary employment responsibilities. With 1PA per week translating to one 12-hour shift monthly, the physician will need to confirm with their service/practice manager that they can be released from other duties in their job plan or increase their number of PAs worked up to 12.

The successful execution of this appointment necessitates time commitments from individuals within a large team to ensure a resilient 365-day service. Compensation for successful candidates from a hospital-based background will align with the pay scale and seniority point of their current professional pay scale, such as Consultant or Specialty Doctor. Candidates holding a CCT in General Practice will be positioned at an appropriate point based on their experience since obtaining their GP CCT on the Rural Practitioner / Emergency Practitioner pay scale.

PICT response team shifts operate for 12 hours, seven days a week.
