Youth Liaison Nurse

We are seeking a skilled and compassionate Youth Liaison Nurse to join our Iechyd Da Youth Health Team. As a key member of our multi-professional team, you will work with vulnerable young people up to 25 years of age across Carmarthenshire, Pembrokeshire, and Ceredigion.
Key Responsibilities- Provide 1:1 support to individual young people, promoting equality of access to health information and advice.
- Deliver health promotion workshops and community events to prevent health inequalities and promote positive health.
- Maintain collaborative working relationships with various agencies working with vulnerable or at-risk young people.
- Represent Hywel Dda University Health Board on health issues for vulnerable children and young people in multi-agency operational groups.
- Registered Nurse/Midwife/Health Visitor/RMN with current registration with the NMC.
- Postgraduate diploma-level knowledge of sexual health, substance misuse, emotional health, and other issues relevant to young people.
- Relevant experience at Band 5/6 level, including working with young people and managing a caseload.
- Teaching qualification or training in relevant subjects.
- Experience working as part of a multi-disciplinary team and in partnership with other agencies.
- Experience working with Looked After Children, children with physical or learning disabilities, and children on the Child Protection Register.
The ability to communicate in Welsh is desirable for this post. If you do not meet the Welsh Language requirements, the Health Board offers learning options and staff support to help you meet these requirements during your employment with us.
