Position Overview

Our Integrated Neighbourhood Teams (INTs) are dedicated to providing care outside of hospital settings, ensuring that individuals receive support in their homes or community clinics. The North and West Bristol INTs focus on delivering these essential services to individuals registered with local General Practitioners.

Senior Clinical Practitioners (SCPs) specializing in Long Term Conditions are highly skilled clinicians with advanced competencies in clinical practice, case management, and collaborative decision-making. They play a crucial role in assisting individuals who exhibit significant reliance on healthcare services. Through specialized support, these individuals can maintain their safety at home while enjoying greater autonomy over their healthcare choices. Effective case management is vital for supporting those with chronic health conditions.

Team Composition

The Locality teams consist of:

  • Senior Clinical Practitioners for Urgent and Long-term Conditions
  • Community Nursing Staff
  • Community Therapy Professionals
  • Paramedics
  • Community Support Personnel
  • Administrative Staff

Key Responsibilities

The primary objective of this role is to operate independently as a specialist practitioner, identifying and managing individuals with complex needs who require intensive community support. This is achieved in collaboration with the Integrated Network Team (INT) and primary care teams to avert unnecessary hospital admissions and ensure timely transitions from hospital to community settings, ultimately enhancing the quality of life for these individuals.

As a senior practitioner, you will deliver services aimed at achieving measurable improvements in healthcare for frail individuals with multiple long-term conditions. You will also provide clinical leadership and guidance to the INT, assisting the Locality Manager and Assistant Locality Manager in achieving outstanding evidence-based clinical outcomes. To ensure organizational and service resilience, you may be required to provide support across various teams or services within the locality and across Sirona services.

This position is not classified as an Advanced Practice role aligned with the HEE Multi-professional Framework for Advancing Practice. These roles are currently under review and may undergo a name change in the near future. This is due to the fact that these positions do not necessitate candidates to operate across all four pillars of Advanced Practice to MSc level and are therefore categorized at AFC Band 7. However, they do require a skill set that operates at or towards MSc level within the Advanced Practice Clinical Pillar. This role will not qualify for training funding for an MSc in Advanced Practice; however, training may be supported based on individual learning needs identified through Sirona's CPD funding application processes.

Sirona care & health actively recruits overseas nursing and therapy professionals through an NHS Trust Supplier and adheres to the NHSE Code of Practice regarding applicants from Red List countries. Consequently, we are unable to accept direct applications for this position from candidates not currently residing in the UK.

Qualifications

Essential

  • Current registration with NMC / HCPC or another relevant professional registration body.
  • Successful completion of a relevant post-graduate Level 3 (or higher) course or equivalent modules, such as disease-specific qualifications, physical assessment, and clinical reasoning.
  • Demonstrated commitment to continuous personal and professional development and the ability to study at the Master's level.
  • Willingness to undertake necessary training for the role, including Non-Medical Prescribing and PADRAP.

Desirable

  • Master's level clinical qualification or progress towards achieving this.
  • Non-Medical Independent & Supplementary Prescriber or willingness to complete the Prescribing Module at Master's Level.

Skills, Knowledge & Experience

Essential

  • Experience in Coaching / Teaching and Assessing.
  • Awareness of local, national, strategic, and operational policy developments and their implications.
  • Understanding of the interface between primary care, secondary care, and service providers.
  • Knowledge of legal considerations related to the care of individuals, including the Data Protection Act, Adult Protection procedures, and the Mental Capacity Act.
