Key Responsibilities- Establish and maintain effective relationships with service users, their relatives, and carers to ensure good communication and understanding of their needs.
- Provide information and assistance to service users and their carers to empower them to make informed decisions about their care and support.
- Work collaboratively with service users and their carers to access identified services and support, promoting independence and well-being.
- Monitor mental health and well-being, identifying and reporting changes to the care team to ensure timely interventions and risk management.
- Encourage social and physical activities to promote service users' physical and mental health.

Requirements- Experience of working with clients with social needs and a sound working knowledge of agencies such as housing and benefits.
- Experience of working with clients with substance misuse issues.
- Good listening skills and ability to deal with complex information shared by service users and care coordinators.
- NVQ level 3 in Health and Social Care or equivalent experience.
- Training to Level 2 in Safeguarding Children and Young People: roles and competencies for Health Care Staff.
