Key Responsibilities
  • Establish and maintain effective relationships with service users, their relatives, and carers to ensure good communication and understanding of their needs.
  • Provide information and assistance to service users and their carers to empower them to make informed decisions about their care and support.
  • Work collaboratively with service users and their carers to access identified services and support, promoting independence and well-being.
  • Monitor mental health and well-being, identifying and reporting changes to the care team to ensure timely interventions and risk management.
  • Encourage social and physical activities to promote service users' physical and mental health.

Requirements
  • Experience of working with clients with social needs and a sound working knowledge of agencies such as housing and benefits.
  • Experience of working with clients with substance misuse issues.
  • Good listening skills and ability to deal with complex information shared by service users and care coordinators.
Desirable Qualifications
  • NVQ level 3 in Health and Social Care or equivalent experience.
  • Training to Level 2 in Safeguarding Children and Young People: roles and competencies for Health Care Staff.
