About This Role
We are seeking a highly motivated and experienced Registered Mental Health Nurse or Mental Health Allied Health Professional to join our All Age Continuing Care and Specialist Placements Team (AACC). You will play a vital role in providing comprehensive care and support to patients originating from North Kent, placed across various geographical locations within England.
Your Responsibilities
- Provide holistic assessments for patients according to the National Framework for NHS Continuing Healthcare and Funded Nursing Care (CHC/FNC).
- Manage complex clinical cases, ensuring adherence to CHC/FNC guidelines and Section 117 Aftercare protocols.
- Develop personalized care and support plans, incorporating Personal Health Budgets (PHBs) and commissioning services for eligible individuals.
- Collaborate with the Associate Director and Assistant Head of AACC to oversee Band 6 nurses and ensure adherence to best practices.
- Contribute to multiagency working environments, effectively communicating with stakeholders involved in patient care.
Essential Skills and Experience
- Relevant graduate/diploma qualification or equivalent level of skill, knowledge, and experience in mental health.
- Post-registration clinical experience in a variety of acute/community mental health settings.
- Experience managing CHC reviews at both local and external independent review panels.
- Full Driver's License
- Proven ability to manage complex cases and navigate challenging situations.
Desirable Skills and Experience
- Experience evaluating mental and physical health needs.
- Evidence of continuing professional development in the field of mental health.
- Demonstrated experience coordinating and delivering outcomes in complex environments.
- Experience supporting change management processes and adapting to evolving healthcare landscapes.
