About the Role

We are seeking a highly skilled and experienced District Nurse Team Leader to join our team at University Hospitals of Morecambe Bay NHS Foundation Trust. As a key member of our Community Nursing Team, you will be responsible for leading and managing a team of nurses to deliver high-quality patient care in the community.

Main Responsibilities
  • Lead and manage a team of nurses to deliver patient care in the community
  • Develop and implement effective care plans to meet the needs of patients
  • Work closely with other healthcare professionals to ensure seamless care delivery
  • Manage a caseload of complex patients and provide clinical leadership to the team
  • Develop and maintain relationships with other healthcare providers to ensure effective care coordination
Requirements
  • Registered Nurse with significant recent community experience
  • Post-graduate education in related areas, such as frailty, falls, or long-term condition management
  • Community specialist practitioner course
  • Minimum 2 years post-qualification experience
Desirable
  • People management skills training
  • Previous management courses
  • Experience of senior position band 6 within DN team
