About the Role:

Creative Support Ltd is seeking a Support Worker to join our Chorley ABI Service team. As a Support Worker, you will play a vital role in providing bespoke adapted supported living services to an individual moving out of a hospital setting.

Key Responsibilities:

  • Provide person-centred support to enable the individual to lead a fulfilling and valued life
  • Develop and implement individualised support plans to promote independent living skills and achieve personal goals
  • Work collaboratively with the individual, their family, and other professionals to ensure a smooth transition to the community
  • Contribute to the development of the service, including identifying and implementing new initiatives to enhance the individual's experience

Requirements:

  • Minimum of twelve months experience of working in a support role
  • Demonstrated ability to work dynamically and adapt to changing circumstances
  • Person-centred values and a 'can-do approach'
  • Reliability and a commitment to providing high-quality support
