Job Summary:

Creative Support Ltd is seeking a dedicated and compassionate Support Worker to join our team in providing adapted living services for an individual with complex needs. As a Support Worker, you will play a vital role in enabling our service user to lead a fulfilling and valued life, participating in his local community, developing independent living skills, and achieving his personal goals.

Key Responsibilities:

  • Provide person-centered support to our service user, promoting his independence and autonomy.
  • Develop and maintain a strong relationship with our service user, understanding his needs, preferences, and goals.
  • Work collaboratively with our team to ensure a seamless transition from hospital to community-based living.
  • Support our service user in developing his skills and interests, such as keeping fit, video games, music, puzzles, bowling, cinema, football, and snooker.
  • Assist our service user in accessing community resources and services, promoting his social inclusion and participation.
  • Contribute to the development and implementation of our service user's personalized support plan.
  • Maintain accurate and up-to-date records of our service user's progress and well-being.

Requirements:

  • Experience working with individuals with complex needs and disabilities.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work a dynamic rota, including evenings and weekends.
  • Ability to lone work and use your own initiative.
