Nurse Practitioner

We are seeking a dedicated and compassionate Registered Nurse to join our care home team at HC-One Ltd. As a Nurse Practitioner, you will have the opportunity to make a meaningful difference in the lives of our residents and their families.
Key Responsibilities- Provide high-quality patient care and support to residents with complex needs
- Develop and implement individualized care plans to meet the unique needs of each resident
- Collaborate with multidisciplinary teams to ensure seamless care delivery
- Conduct regular assessments and evaluations to monitor resident progress and adjust care plans as needed
- Provide education and support to residents and their families on health-related topics
- Valid Pin number and registration with the NMC
- Experience working in a care home environment
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
