We are seeking an enthusiastic and dedicated professional to join our multi-disciplinary team as an Adult Mental Health Senior Practitioner. As a key member of our team, you will play a vital role in supporting the assessment and care planning for our service users.

Main Responsibilities
  1. To be an active member of the team, working collaboratively with colleagues to deliver high-quality care and support to our service users.
  2. To act as care coordinator to an agreed caseload of service users with severe and enduring mental illness, providing expert guidance and support to ensure the best possible outcomes.

Person Specification

Essential Qualifications

  • Registered Nurse, Social Worker, Mental Health Practitioner, or equivalent qualification
  • Significant post-qualifying experience in mental health care
  • Computer skills and ability to use electronic patient records
  • Mentorship course and clinical supervision training
