Position Overview
The role of the Community Palliative Care Nurse is to support the Head of Outreach Services and the Senior Staff Nurse in delivering compassionate, comprehensive, and individualized care for individuals facing life-limiting illnesses and their families within our community.
Key Responsibilities
The successful candidate will be accountable for providing exceptional standards of specialized palliative care to assist patients and their families. Essential skills include effective communication, a strong commitment to care values, and a collaborative team spirit.
The position entails conducting holistic nursing evaluations (including home assessments), coordinating with both internal and external healthcare professionals, facilitating support groups (including virtual options), and contributing to the ongoing development of Outreach Services.
This role offers a unique opportunity to significantly impact the lives of those in need. The post holder will be expected to demonstrate adaptability and creativity in their caregiving approach, maximizing holistic interventions across outreach initiatives and partnership collaborations, embodying our core values that emphasize the importance of every interaction.
Please note that the candidate must possess a valid UK driving license, own a vehicle, and demonstrate proficient technical and IT skills.

Willowbrook Hospice is a dedicated independent facility established to provide specialized palliative care for adults aged 18 and over, serving the local community. Our mission is to enhance the quality of life for individuals with life-limiting or terminal conditions, ensuring they can live fully and meaningfully.
At Willowbrook Hospice, we prioritize not only the physical needs of our patients but also their emotional, spiritual, and social well-being. We extend our support to caregivers, family members, and close friends throughout the illness and during bereavement.
Job Duties
The Community Palliative Care Nurse will assist the Head of Outreach Services and the Senior Staff Nurse in delivering a responsive and holistic service tailored to the unique needs of individuals with life-limiting conditions.
Responsibilities include:
- Conducting comprehensive nursing assessments
- Providing family support
- Collaborating with healthcare professionals
- Facilitating support groups
- Contributing to the development of Outreach Services
All duties will be performed in alignment with the Hospice's philosophy, ensuring public trust and confidence.
Qualifications
Essential:
- First Level registration
- Recent professional development
Desirable:
- Foundation course in specialist palliative care/end-of-life care
- Spiritual Care Training
- Bereavement Training
- Complementary Therapies
Experience
Essential:
- Genuine interest in Palliative Care
- Demonstrated clinical expertise in symptom management
- Commitment to ongoing professional development
- Current knowledge in general nursing care
- Experience in a multi-disciplinary team
- Understanding of palliative nursing principles
- Knowledge of clinical governance
- Able to effectively deputize for the Senior Staff Nurse
Desirable:
- 12 months post-registration experience as a Staff Nurse
- Experience in palliative care
