Exciting Opportunity for a Senior Nurse in Early Intervention

Birmingham Community Healthcare NHS Foundation Trust is seeking an experienced Senior Nurse to join our Early Intervention Community Team. As a key member of our team, you will play a vital role in delivering high-quality care to patients in their own homes.

Main Responsibilities
  • Work collaboratively with multidisciplinary teams to provide comprehensive assessments and develop individualized care plans.
  • Lead and mentor less experienced staff members to ensure seamless delivery of care.
  • Contribute to the development and implementation of quality initiatives, such as falls prevention programs.
  • Collaborate with therapists to identify and implement special adaptations and equipment to support patient care.

Requirements
  • Registered Nurse Level 1 (Adult)
  • Evidence of up to date post-registration education relevant to the role
  • Degree in a relevant subject
  • Nurse prescriber
Experience
  • Significant experience as a registered nurse, including working in a rehabilitation setting or care of the older adult
  • Experience working as a registered nurse in the community
  • Significant experience of a multidisciplinary approach to care
  • Comprehensive assessment skills and knowledge of the needs of the older person, including risk management
